YSK medical offices are required to keep a log of who accessed your medical record and when, and this log is available to you at your request.

More and more practices are merging their medical records into larger electronic medical record systems like EPIC, which is good if you're on vacation and break your leg -- the doctor can easily bring up your medical chart from your primary care back home. However, think of all the employees in a large health system. Anyone who knows you could be tempted to open your chart just out of curiosity, and it is highly unlikely they'll get caught.

If you suspect that has ever happened, then just know you have a right to contact your office and see who accessed your chart and when, which they are required to keep track of as part of HIPAA.
